The following providers and pharmacies are NOT in your AHPT Network. Some of these may appear to be in-network in a provider search, but will be processed as out of network.
Out-of-network Providers
Out-of-Network Coverage
For any charges incurred outside of your network, you will be responsible for the difference between what the coverage allows and the provider’s charges. Amounts exceeding Plan limitation are non-covered, and you may be balance billed:
Physician’s charges will be reimbursed 130% of the provider’s Medicare rate.
Outpatient laboratory will be reimbursed 100% of the provider’s Medicare rate.
